Is it safe to take arginine with nitrates?
It is not safe to assume so: arginine with nitrates, such as nitroglycerin or isosorbide, might cause blood pressure to become too low.
Mayo Clinic says the combination with nitrates and with sildenafil "might cause your blood pressure to become too low" (Mayo Clinic, L-arginine). Nitrates widen blood vessels on purpose, and arginine is sold as a nitric-oxide building block, so the two effects could stack.
Who should not take arginine at all?
People who have recently had a heart attack should not take arginine supplements without medical advice.
In the VINTAGE MI trial, 6 of 153 patients given arginine after a heart attack died, and none on placebo did (Schulman 2006, VINTAGE MI). That trial used gram-level doses, but it is the reason the warning exists.
